About this download

Every provider, authorization, appointment and phone call for your child's care — in one place you control.

Most parents say they wish someone would coordinate their child's care. Until a clinic gives you that person, this binder is how you become them — without holding it all in your head.

Print it, three-hole punch it, keep it where the phone calls happen. If it happened on the phone, in a waiting room, or in an email, it goes on a page: names, dates, reference numbers, what they said. Three months from now, when an authorization lapses or someone says "we never received that," you open to the page and read it back.

Thirteen sections, 20 pages (PDF, US Letter) — reprint any page as often as you need:

  • Our team — everyone involved, and the best way to reach each one
  • About my child — the one-page handoff for a new provider or babysitter
  • Insurance at a glance, and the four questions to ask the plan once
  • Authorization tracker with renewal reminders
  • Call log and appointment log (two pages each)
  • Questions for the next visit, with space for the answer
  • "What changed this month" — three months of observation pages
  • Medication & changes log
  • School & IEP (ARD) page — the bridge between clinic and school
  • Year at a glance for the dates that quietly end services
  • Documents: what we have and where

Organize, understand, communicate. The plan stays the BCBA's.

Before you buy

How do I get the files?

Shopify emails a download link within a minute of checkout. The link stays in that email, so keep it. If it doesn't arrive, check spam, then write to us using the footer.

Can I edit it?

Printable PDFs print as-is. Editable items open in Canva or Google Docs, as listed under what you receive.

Is this treatment advice?

No. Everything on the shelf is a template or educational material. Your child's team decides what fits your child.

Is it right for my situation?

Read 'who it may help' and the page previews. If it isn't a fit, don't buy it; the free ABC logger is here either way.

Refunds?

Downloads can't be returned, but if something is broken or not as described, write to us and we'll make it right.

Because the tasks go together, not because of a sales target.
